Working papers will be completed ONLINE at myworkingpapers.nj.gov Thanks to a new law, the working papers process will be online and streamlined. Rather than going through local schools, younger workers will simply visit this site and be walked through a few easy steps. Please print the working papers linked BELOW.
2. Complete Section A (Minor's Personal Information) completely. Parent Signature required. 3. Section B (Employment Information) ALL info must be completed and signed by the EMPLOYER. Employer must indicate number of hours and/or starting/stopping times for each day, with total hours for the week. 4. Section C (Physician's Certification). Your personal physician must complete this section including signature and date. 5. Section D (Proof of Age). You must email/scan a copy of the birth certificate, baptismal certificate or government issued passport. 6. Section E (School Record). Fill in 'Last Grade Completed*' ONLY. Principal signature will be completed by the school. (*example: if you are a current 11th grader, you would fill in 10 for Last Grade Completed) 7. Section F (Issuing Officer Certification). Student signature and date required. The rest of this section will be completed by the school, only after ALL other information is complete. The school is the last step! 8. Once all required information is complete. Email a clear photo or scan of the working papers AND proof of age (see step 5 above) to [email protected] Completed working papers will be processed and an electronic copy returned to you.
